What is Trigger Finger?

This is one of the most common questions we hear.

Think of the tendons that bend your fingers as lines of rope running through a series of tight pulleys that hold them to the bone. Trigger finger happens when the first pulley at the base of your finger (the A1 pulley) becomes too thick and tight. The tendon can no longer glide smoothly through it. It catches, and then with more force, it pops through with a painful click or snap.

This is what causes the classic symptoms:

  • A painful clicking or popping sensation when you move your finger.
  • The finger getting stuck or ‘locked’ in a bent position, which you might have to straighten with your other hand.
  • A tender lump in your palm at the base of the affected finger.
  • Stiffness and pain, especially first thing in the morning.

How Do I Know If I Have Trigger Finger?

Getting the right diagnosis is the first and most important step.
When you see a specialist like me, I can diagnose trigger finger simply by listening to your story and examining your hand. The diagnosis is usually obvious from the classic locking and the tender lump in your palm. No complicated scans are needed. I can perform an ultrasound scan in the clinic myself, which confirms the diagnosis instantly and allows us to see the problem right there and then, making it extra convenient for you.

What’s the Best Surgery for Trigger Finger?

For many of my patients, the thought of having a cut on their palm is a real concern. You might worry about the pain, the scar, and the time off work. That’s why I’m proud to offer a state-of-the-art 'keyhole' procedure called ultrasound-guided trigger finger release. I am one of only a handful of surgeons in the UK, and the first in Hampshire, to offer this advanced technique in a clinic setting.

How is it different from traditional surgery?

Traditional open surgery involves an incision in your palm about 1-2 cm long to see and cut the tight pulley. While effective, this means cutting the sensitive skin of your palm simply to get to the structure underneath.
With ultrasound-guided release, we only need to make a tiny keyhole incision—just 1-2 millimetres long. Using a real-time ultrasound scanner to see inside your hand, I can guide a special, tiny instrument to precisely divide that tight pulley from the inside, without disturbing the surrounding tissues. The tiny entry point doesn't even need a stitch, just a small plaster.

The difference is significant. We are only cutting the structure that is causing the problem.

Let us tell you about John, a 62-year-old retired gentleman from Southampton who is a passionate golfer. His ring finger kept locking painfully every time he gripped his club, ruining his game. He’d had an injection which worked for a while, but the problem came back.

We performed an ultrasound-guided release in the clinic, which took about five minutes. He went home with a small plaster, was able to chip and putt in his garden the next day, and was back playing a full 18 holes within the week. At his follow-up, he told me it was "like magic" and he'd forgotten all about the procedure.
